Insurance
For chiropractic care, we bill Medicare Insurance and most other insurance plans. If you are unsure how your insurance works, or if you have questions about your chiropractic coverage, give us a call.
Payment
Payment is due at the time that services are rendered. This includes co-payment and deductibles (balances not paid by your insurance). For your convenience, we accept cash, Visa and MasterCard. We will bill your insurance for you, however, financial responsibility remains with the patient.
Below you will find our intake form. Please feel free to print and complete the form before your first visit with us. While we do not require that you complete this paperwork prior to visiting our practice, this will save you and our staff time when you come in for your appointment.
